ABSTRACT

Tecnologia: Enoxaparina comparada à profilaxia mecânica e/ou outros medicamentos disponíveis ou não no SUS. Indicação: Profilaxia de Tromboembolismo Venoso (TEV) em pacientes submetidos a cirurgia de abdome, pelve e varizes. Pergunta: Há superioridade em eficácia e segurança da enoxaparina (heparina de baixo peso molecular - HBPM) comparada à profilaxia mecânica e a outros medicamentos disponíveis ou não no SUS para prevenção de TEV em pacientes acima de 18 anos, não gestantes, em pós-operatório de cirurgias eletivas de abdome, pelve e varizes? Métodos: Revisão rápida de evidências (overview) de revisões sistemáticas, com levantamento bibliográfico realizado na base de dados PUBMED, utilizando estratégia estruturada de busca. A qualidade metodológica das revisões sistemáticas foi avaliada com AMSTAR-2 (Assessing the Methodological Quality of Systematic Reviews). Resultados: Foram selecionadas quatro e incluídas duas revisões sistemáticas com metanálise. Conclusão: HBPM no pós-operatório de cirurgia abdominal e pelve reduziu a incidência de TEV geral e TEV sintomático, sem aumentar risco de sangramento e mortalidade. Nas cirurgias de veias varicosas, foi observado uma redução de todos os eventos trombóticos e risco de TVP, sem aumentar risco de sangramento


Technology: Enoxaparin compared to mechanical prophylaxis and/or other drugs available or not in the SUS. Indication: Prophylaxis of Venous Thromboembolism (VTE) in patients undergoing surgery of the abdomen, pelvis and varicose veins. Question: There is superiority in efficacy and safety of enoxaparin, compared to mechanical prophylaxis and other drugs available or not in the SUS, for the prevention of VTE for patients over 18 years old, non-pregnant in the postoperative period of elective surgeries of the abdomen, pelvis and varicose veins? Methods: Rapid review of evidence (overview) from systematic reviews, with a bibliographic search in the PUBMED database, using a structured strategy. The methodological quality of systematic reviews was assessed with AMSTAR-2 (Methodological Quality Assessment of Systematic Reviews). Results: Four were selected and two systematic reviews with meta-analysis were included. Conclusion: LMWH in the postoperative period of abdominal and pelvic surgery reduced the incidence of general VTE and symptomatic VTE, without increasing the risk of bleeding and mortality. In varicose vein surgeries, a reduction in all thrombotic events and risk of DVT was observed, without increasing the risk of bleeding

Resumo Contexto Apesar de todo o investimento na profilaxia primária do tromboembolismo venoso (TEV) em pacientes cirúrgicos nos últimos anos, ainda não existem diretrizes específicas para aqueles que serão submetidos a procedimentos para tratamento de varizes de membros inferiores. Objetivos Avaliar o perfil de conduta de profilaxia do TEV pelos cirurgiões vasculares brasileiros para procedimentos de tratamento de varizes de membros inferiores. Métodos Pesquisa de levantamento por envio de questionário eletrônico a cirurgiões vasculares brasileiros. Os respondentes foram divididos entre os que realizam tratamento de veias safenas por cirurgia convencional e os que realizam termoablação para fim de comparação entre os grupos. Resultados Entre os 765 respondentes, o tratamento de escolha das veias safenas foi a cirurgia convencional para 405 (53%), espuma ecoguiada para 44 (6%) e termoablação (endolaser ou radiofrequência) para 199 (26%). Os cirurgiões que realizam termoablação prescrevem mais farmacoprofilaxia após o procedimento que aqueles que preferem cirurgia convencional (67/199, 34% vs. 112/405, 28%; p = 0,002). O grupo termoablação estratifica o paciente quanto ao risco de TEV com mais frequência que o grupo cirurgia convencional (102/199, 51% vs. 179/405, 44%; p =0,004). Ambos os grupos usam mais frequentemente enoxaparina como medicação para profilaxia, porém o grupo termoablação usa mais anticoagulantes orais diretos proporcionalmente que o grupo cirurgia convencional (26% vs. 10%, p < 0,001). Conclusões Cirurgiões vasculares brasileiros que fizeram o tratamento de veias safenas por termoablação prescrevem farmacoprofilaxia com maior frequência e por um período mais prolongado do que os que realizaram o tratamento por cirurgia convencional.


Abstract Background Despite all the investment in primary venous thromboembolism (VTE) prophylaxis for surgical patients in recent years, there are still no specific guidelines for those who undergo procedures to treat lower limb varicose veins. Objectives To evaluate the profile of VTE prophylaxis practices among Brazilian vascular surgeons conducting lower limb varicose vein procedures. Methods Survey design, sending an electronic questionnaire to Brazilian vascular surgeons. Respondents were divided between those who perform saphenous vein treatment with conventional surgery and those who perform thermoablation for the purpose of comparison between groups. Results Of 765 respondents, 405 (53%) treat saphenous veins with conventional surgery for, 44 (6%) with foam, and 199 (26%) with thermoablation (endolaser or radiofrequency). Surgeons who perform thermoablation prescribed more pharmacoprophylaxis after varicose vein surgery than those who perform conventional surgery (67/199, 34% vs. 112/405, 28%; p = 0.002). The thermoablation group stratifies patients for thromboembolism risk more frequently than the conventional surgery group (102/199, 51% vs. 179/405, 44%; p = 0.004). Both groups use enoxaparin as the most frequent drug for prophylaxis, but the thermoablation group uses proportionally more direct oral anticoagulants than the conventional surgery group (26% vs. 10%, p<0.001). Conclusions Brazilian vascular surgeons who perform saphenous vein treatment by thermoablation prescribe pharmacoprophylaxis more frequently and for a longer period than those who use conventional surgery.

Resumo Contexto Pacientes com doença venosa crônica avançada são mais propensos a exigir outros procedimentos para recidiva de veias varicosas. Ainda não está estabelecido se a gravidade da insuficiência venosa é um fator que influencia a taxa de oclusão de veias safenas tratadas por endolaser. Objetivos Analisar a taxa de oclusão dos segmentos venosos tratados com endolaser e correlacionar com o Venous Clinical Severity Score (VCSS) e a classificação Clínica-Etiológica-Anatômica-Patológica (CEAP) dos pacientes. Métodos Análise retrospectiva de coorte de pacientes operados com endolaser 1.470 nm entre novembro/2012 a março/2020. Foram realizadas estatística descritiva e curva de sobrevida de Kaplan-Meier com regressão de Cox para grupos de VCSS e CEAP. Resultados Foram analisados 170 pacientes e 180 segmentos venosos; a idade média foi de 44,3 ± 9,2, sendo a maioria do sexo feminino (71%). A densidade de energia média utilizada na veia safena magna foi 49,2 ± 8,3 J/cm. As principais complicações foram dor no trajeto da safena (12,2%) e parestesias após 6 meses (17,2%). Não houve diferença na taxa de oclusão venosa entre grupos com VCSS ≤ 7 e VCSS > 7 (p = 0,067). O grupo de pacientes com CEAP agrupada C4-C5-C6 teve taxa de oclusão menor em relação ao grupo C2-C3 [hazard ratio (HR) = 3,22; intervalo de confiança (IC) 1,85, 5,61; p = 0,001]. Conclusões As taxas de oclusão de segmentos venosos tratados com endolaser foram menores na presença de classificações CEAP avançadas. Nesses pacientes, provavelmente deve-se despender mais energia para o tratamento eficaz das safenas.


Abstract Background Patients with advanced chronic venous disease are more likely to need additional procedures for relapsed varicose veins. It has not yet been established whether severity of venous insufficiency is a factor that influences the occlusion rate of saphenous veins treated with endolasers. Objectives To analyze occlusion rate of venous segments treated with endolaser and correlate it with patients' Venous Clinical Severity Score (VCSS) and Clinical-Etiological-Anatomical-Pathological (CEAP) classification. Methods Retrospective analysis of a cohort of patients operated using a 1,470 nm endolaser from November 2012 to March 2020. Descriptive statistics were calculated and Kaplan-Meier survival curves were plotted with Cox regression for groups stratified by VCSS and CEAP. Results A total of 180 venous segments were analyzed in 170 patients. Mean age was 44.3 ± 9.2 and the majority of patients were female (71%). Mean energy density used in the great saphenous vein was 49.2 ± 8.3 J/cm. The most common complications were pain along the course of the saphenous vein (12.2%) and paresthesias at 6 months (17.2%). There was no difference in venous occlusion rate between groups with VCSS ≤ 7 and VCSS > 7 (p = 0.067). A group of patients classified as CEAP classes C4, C5, or C6 had a lower occlusion rate than a group at classes C2 or C3 (hazard ratio [HR] = 3.22; confidence interval [CI] 1.85, 5.61; p = 0.001]. Conclusions The occlusion rates of venous segments treated with endolaser were lower in patients with higher CEAP classes. It is probably necessary to use more energy in these patients to achieve effective treatment of saphenous veins.

ABSTRACT

OBJECTIVE@#To explore the technical details and short-term effects of radiofrequency obliteration of varicose veins of lower extremities guided by combined venography and ultrasound.@*METHODS@#Thirty-seven patients with varicose veins of lower extremities were treated with radiofrequency obliteration using Olympus Celon RFiTT® under combined guidance of venography and ultrasound. The indications included varicose veins of lower extremities and reflux of the great saphenous vein confirmed by ultrasound. The contraindications included deep vein thrombosis, cardiac pacemaker, severe cardio- and cerebrovascular diseases or coagulation disorders. Under ultrasound guidance, the saphenous vein around knee level was punctured using a 21G needle, and a 7F sheath was introduced. Through the sheath a venography was made, and an Olympus Celon ProCurve radiofrequency catheter was inserted and advanced to the great saphenous vein under road map, and the catheter tip was positioned at the point 2 cm below the sapheno-femoral junction. The swelling anesthesia was made under ultrasound guidance. Then the radiofrequency obliteration was performed with pressing of the treatment section. The venography was repeated to ensure optimal outcomes. If necessary the radiofrequency obliteration could be repeated once to twice. After that the superficial varicose veins were stripping by small incisions under local anesthesia. After operation, medical decompression stocking was utilized immediately and sustained for three months. The clinical data, intraoperative radiation dose, exposure time and short-term effects were retrospectively analyzed.@*RESULTS@#After the operation, all the patients walked out of the operating room by themselves. The success rate of operation was 100%. The intraoperative radiation dose was 1.78-10.12 mGy (mean 6.56 mGy), and the exposure time was 61-448 s (mean 161 s). By 3 months follow-up, the symptoms were alleviated in all the 37 patients, and the occlusion rate was 100%. No complications such as skin burns, ecchymosis and deep venous thrombosis were found.@*CONCLUSION@#The short-term effects of radiofrequency obliteration using Olympus Celon RFiTT® system in a manner of twice fixed point followed by once reciprocating radiofrequency were satisfactory. Radiofrequency obliteration of great saphenous veins guided by venography and ultrasound has not only the advantages of minimal trauma and rapid recovery, but also the advantages of accurate location, exact effect and avoidance of complications.

Resumo Contexto Atualmente, recomenda-se como primeira opção cirúrgica de varizes a termoablação da veia safena; porém, esse procedimento não é realizado pelo Sistema Único de Saúde do Brasil. Como forma de incluir melhores resultados, técnicas cirúrgicas esforçam-se para mimetizar as novas tecnologias sem seus custos, sendo a principal delas a realização da safenectomia convencional sem ligadura das suas tributárias. Objetivos Avaliar a evolução do coto residual após safenectomia sem ligadura alta da junção safeno-femoral associada à invaginação do mesmo, assim como avaliar o comportamento das veias acessórias anterior/posterior. Métodos Estudo prospectivo e intervencionista. Foram operados 52 membros pela técnica de safenectomia sem ligadura alta da junção safeno-femoral seguida da invaginação do coto residual. Os pacientes foram avaliados no pré e pós-operatório (7 dias, 3, 6 e 12 meses) através de ultrassonografia vascular com Doppler para análise de diâmetro e extensão do coto residual, diâmetro e refluxo na veia acessória anterior/posterior e presença de neovascularização. A análise estatística foi realizada por média, desvio padrão, mediana, valor mínimo e máximo, frequências e percentuais, teste de Fisher e bimodal. Resultados Evidenciou-se um efeito significativo do tempo sobre a medida de diâmetro (p < 0,001) e da extensão (p = 0,002) do coto residual, porém o mesmo não foi observado quanto ao diâmetro (p = 0,355) ou refluxo na veia acessória anterior. Foi identificada neovascularização em 7 (14,3%) membros. Conclusões Após a utilização da técnica descrita, o coto residual apresentou retração e diminuição do seu diâmetro no período de 1 ano e não transmitiu refluxo para veia acessória. As taxas de neovascularização foram condizentes com a literatura.


Abstract Background Currently, the first-choice option recommended for varicose vein surgery is thermal ablation of the saphenous vein, but this procedure is not available on the Brazilian National Health Service (SUS - Sistema Único de Saúde). In an effort to improve results, surgical techniques have been developed to mimic the new technologies, without their high costs. The most prominent such method involves conventional saphenectomy, without ligation of tributaries. Objectives To assess progression of the residual stump after saphenectomy without high ligation of the saphenofemoral junction but with stump invagination and to assess the behavior of anterior/posterior accessory veins. Methods Prospective intervention study. A total of 52 limbs were treated with saphenectomy without high ligation of the saphenofemoral junction followed by invagination of the residual stump. Patients were assessed preoperatively and at 7 days, and 3, 6, and 12 months postoperatively using vascular ultrasonography with Doppler to analyze the length of the residual stump, the diameters of the residual stump and the anterior/posterior accessory vein, reflux in the accessory vein, and presence of neovascularization. Statistical analysis involved calculation of means, standard deviations, medians, minimum and maximum values, frequencies, and percentages, and Fisher's test and the binomial test. Results There was evidence of a significant time effect on residual stump diameter (p < 0.001) and length (p = 0.002), but the same was not observed with relation to diameter (p = 0.355) or reflux of the anterior accessory vein. Neovascularization was found in 7 (14.3%) limbs. Conclusions After use of the technique described, the residual stump retracted, its diameter reduced over the 1 year postoperative period, and it did not transfer reflux to the accessory vein. Neovascularization rates were in line with the literature.

Resumo Contexto Existem diversas formas de tratamento de varizes de membros inferiores. Entre elas, destaca-se o uso do laser diodo de 1.470 nm. Essa técnica proporciona aos pacientes uma cirurgia em regime ambulatorial, com retorno precoce à atividade ocupacional, bom resultado estético e baixo índice de complicações. No entanto, ainda se discute exaustivamente variáveis como comprimento de onda do laser, potência aplicada em cada área, tipo de fibra, necessidade ou não de tumescência e densidade de energia endovenosa linear. Objetivos Analisar os resultados do tratamento da insuficiência venosa superficial com laser diodo de 1.470 nm. Métodos Estudo retrospectivo, realizado em uma clínica privada de um hospital privado em Florianópolis a partir de dados colhidos prospectivamente. As amostras eram de 287 pacientes submetidos à cirurgia para tratamento da insuficiência venosa superficial com laser diodo de 1.470 nm, de janeiro de 2016 a dezembro de 2018, totalizando 358 veias safenas magnas e 84 veias safenas parvas tratadas. Resultados A taxa de oclusão total após 12 meses de cirurgia foi de 94,4%, com densidade de energia endovenosa linear média de 45,90 J/cm nas veias safenas magnas e de 96,4% com densidade de energia endovenosa linear média de 44,07 J/cm nas veias safenas parvas. Conclusões No período acompanhado, o laser diodo de 1.470 nm mostrou-se um tratamento seguro, muito efetivo e com baixas taxas de complicações (dor, edema, equimose, trombose venosa profunda e trombose induzida pelo calor endovenoso).


Abstract Background There are several ways to treat varicose veins of the lower limbs, among which use of 1470nm diode lasers stands out. This technique can be used to treat patients in outpatient settings, with early return to work, good esthetic results, and low rates of complications. However, variables such as the laser wavelength, the power administered in each area, the type of fiber, and the linear intravenous energy density (LEED) are still extensively discussed. Objectives To analyze the results of superficial venous insufficiency treatment with a 1470nm diode laser. Methods Retrospective study conducted at a private clinic in a private hospital in Florianopolis, based on a database collected prospectively. The sample comprised 287 patients who underwent surgery to treat superficial venous insufficiency with 1470nm diode laser, from January 2016 to December 2018, totaling 358 great saphenous veins (GSVs) and 84 small saphenous veins (SSVs) treated. Results The total occlusion rates after 12 months of surgery were 94.4% in the GSVs, with an average LEED of 45.90 J/cm, and 96.4% in the SSVs, with an average LEED of 44.07 J/cm. Conclusions During the follow-up period, the 1470nm diode laser proved to be a safe treatment, with great efficacy and low rates of complications (pain, edema, bruising, deep vein thrombosis, and endothermal heat-induced thrombosis - EHIT).

Introducción: La neuralgia del nervio safeno interno, possafenectomía, constituye la expresión natural de una lesión nerviosa de tipo troncular periférico y constituye un síndrome clínico frecuente pero poco estudiado desde todos los puntos de vista. Objetivo: Valorar la neuralgia del safeno interno en los pacientes operados de várices esenciales en miembros inferiores. Método: Se realizó un estudio descriptivo- retrospectivo en 60 pacientes operados (stripping o flebo-extracción) en el servicio de Angiología y Cirugía Vascular del Hospital Docente Clinicoquirúrgico "General Freyre de Andrade" con el diagnóstico de várices esenciales en los miembros inferiores y presencia de neuralgia del nervio safeno interno. El estudio se realizó de enero a diciembre de 2017. Las variables estudiadas fueron: sexo, localización de la safenectomía (izquierda, derecha, bilateral), presencia de neuralgia en el nervio safeno interno. Se trabajó con valores de frecuencias absolutas y relativas. Resultados: 40 por ciento de las safenectomías fueron bilaterales; 31,6 por ciento en miembro inferior derecho y 28,3 por ciento en el miembro inferior izquierdo. El 80 por ciento eran mujeres (25 por ciento lado izquierdo, 25 por ciento lado derecho y 50 por ciento bilateral) y el 20 por ciento hombres (58,3 por ciento lado derecho y 41,7 por ciento lado izquierdo). Pacientes con neuralgia del safeno interno 23,3 por ciento (41,7 por ciento hombres y 18,8 por ciento mujeres). Conclusión: El reconocimiento adecuado de la neuralgia del safeno interno permite excluir otras complicaciones de tipo vascular y establecer medidas profilácticas para evitarla(AU)


Introduction: Post-saphenectomy neuralgia of the internal saphenous nerve is the natural expression of a peripheral truncal nerve injury and constitutes a frequent clinical syndrome; however, it is rarely studied, from all points of view. Objective: To assess neuralgia of the internal saphenous nerve in patients operated on for essential varicose veins of the lower limbs. Method: A descriptive-retrospective study was carried out in 60 surgically intervened patients (stripping or phlebo-extraction) in the angiology and vascular surgery service of Freyre de Andrade Clinical-Surgical Teaching Hospital for a diagnosis of essential varicose veins of the lower limbs and manifestation of neuralgia of the internal saphenous nerve. The study was carried out from January to December 2017. The variables studied were sex, location of the saphenectomy (left, right, or bilateral), and manifestation of neuralgia in the internal saphenous nerve. We worked with absolute and relative frequency values. Results: 40 percent of the saphenectomies were bilateral, 31.6 percent were performed in the lower right limb, and 28.3percent corresponded to the lower left limb. 80 percent were women (left side: 25 percent, right side: 25 percent, and bilateral: 50 percent) and 20 percent were men (right side: 58.3 percent, and left side: 41.7 percent). Patients with neuralgia of the internal saphenous never accounted for 23.3 percent (41.7 percent were men and 18.8 percent were women). Conclusion: The correct identification of neuralgia of the internal saphenous nerve allows to exclude other vascular complications and to establish prophylactic measures to avoid it(AU)

Resumo Contexto As varizes dos membros inferiores representam uma das doenças mais prevalentes na população mundial e resultam em grande impacto na qualidade de vida dos pacientes devido às limitações nas atividades diárias e no desempenho funcional. Vários autores têm ressaltado a importância da avaliação da qualidade de vida na abordagem de pacientes portadores de doença venosa crônica. Objetivos Avaliar a qualidade de vida antes e após o tratamento cirúrgico de pacientes portadores de varizes dos membros inferiores. Métodos Através de estudo de antes e depois, foram avaliados 92 portadores de varizes dos membros inferiores submetidos a tratamento cirúrgico. Os pacientes foram divididos em grupos de acordo com a faixa etária e a classificação clínica CEAP. A qualidade de vida foi avaliada através do questionário VEINES QOL/SYM, aplicado no pré-operatório e 60 dias após a operação. Resultados Do total de 92 indivíduos analisados, 82,6% (76) eram mulheres. Com relação à idade, a média foi de 45,7±12,11. A classe CEAP 2 foi a mais encontrada, em 57,6% dos pacientes. Foi encontrada diferença significativa entre os escores antes e após a cirurgia para os pacientes na faixa etária entre 30 e 40 anos. Não houve diferença entre os escores nos momentos pré e pós-operatório entre os grupos CEAP. Conclusões Não foi encontrada diferença na qualidade de vida antes e após a cirurgia na maioria dos pacientes do estudo.


Abstract Background Lower limb varicose veins are one of the most prevalent diseases in the global population. The disease is chronic and has a great impact on patients' quality of life, limiting daily activities and functional performance. Several authors have emphasized the importance of including quality of life assessment in management of patients with chronic venous disease. Objectives To evaluate quality of life before and after surgical treatment of patients with varicose veins. Methods A before and after study design was employed. Ninety-two people with varicose veins of the lower limbs were treated surgically. Patients were divided into subsets according to age and CEAP clinical classification. Quality of life was assessed using the VEINES QOL/SYM questionnaire, administered during the preoperative period and 60 days after the operation. Results The sample comprised 92 subjects, 82.6% (76) of whom were women and mean age was 45.7±12.11 years. CEAP class 2 was the most frequent clinical classification, in 57.6% of patients. There was a significant difference in scores before and after surgery among patients aged from 30 to 40 years. There was no difference between preoperative and postoperative scores between different CEAP groups. Conclusions No difference in quality of life was observed after surgery in most of the patients in the present study.

Les auteurs rapportent un cas d'un syndrome de Klippel­Trenaunayrévélé par des malformations variqueuses systématisées et une ostéodystrophie avec un reflux ostial important du membre inférieur droit au CHU de Bouaké. La chirurgie classique a consisté à un éveinage étendu associé à une crossectomie droite. L'examen histologique des lacis veineux était favorable à l'existence d'un angiome. Les suites opératoires ont été simples. Après un recul de 14 mois, le patient présente une myalgie intermittente survenant à l'effort de marche. Cette observation pose le problème de l'évolution postopératoire des varices congénitales

ABSTRACT

CHIVA (Cure Conservatrice et Hemodynamique de l'Insufficience Veineuse en Ambulatoire) is a type of operation for varicose veins that avoids destroying the saphenous vein and collaterals. We report a case of CHIVA treatment of two saphenous veins to spare these veins. The patient previously had a normal great saphenous vein stripped in error in a wrong-site surgery, while two saphenous veins that did have reflux were not operated. The patient was symptomatic and we performed a CHIVA operation on the left great and right small saphenous veins. The postoperative period was uneventful and both aesthetic and clinical results were satisfactory. This case illustrates that saphenous-sparing procedures can play an important role in treatment of chronic venous insufficiency. Additionally, most safe surgery protocols do not adequately cover varicose veins operations. Routine use of duplex scanning by the surgical team could prevent problems related to the operation site


Cure conservatrice et hemodynamique de l'insufficience veineuse en ambulatoire (CHIVA) é um tipo de cirurgia de varizes que evita a destruição da veia safena e colaterais. Este relato apresenta uma paciente que foi submetida a CHIVA em duas safenas para poupá-las. A paciente teve uma safena magna normal retirada em uma cirurgia no sítio cirúrgico errado, as safenas com refluxo foram mantidas, e uma normal foi ressecada. A paciente estava sintomática e foi realizada CHIVA na safena parva direita e na magna esquerda. O pós-operatório transcorreu bem com resultado clínico e estético satisfatório. Esse caso mostra que cirurgias que poupam a safena têm papel importante no tratamento da insuficiência venosa crônica. Além disso, os protocolos de cirurgia segura não cobrem adequadamente as cirurgias de varizes devido a duas safenas possíveis e por serem frequentemente cirurgias bilaterais. A realização de eco-Doppler rotineiramente pela equipe cirúrgica pode prevenir problemas relacionados ao sítio operatório

SUMMARY OBJECTIVE This study aims to correlate the demographic data, different clinical degrees of chronic venous insufficiency (CEAP), ultrasound findings of saphenofemoral junction (SFJ) reflux, and anatomopathological findings of the proximal segment of the great saphenous vein (GSV) extracted from patients with primary chronic venous insufficiency (CVI) submitted to stripping of the great saphenous vein for the treatment of lower limb varicose. METHOD This is a prospective study of 84 patients (110 limbs) who were submitted to the stripping of the great saphenous vein for the treatment of varicose veins of the lower limbs, who were evaluated for CEAP clinical classification, the presence of reflux at the SFJ with Doppler ultrasonography, and histopathological changes. We study the relationship between the histopathological findings of the proximal GSV withdrawal of patients with CVI with a normal GSV control group from cadavers. RESULTS The mean age of the patients was higher in the advanced CEAPS categories when comparing C2 (46,1 years) with C4 (55,7 years) and C5-6(66 years), as well as C3 patients (50,6 years) with C5-6 patients. The normal GSV wall thickness (mean 839,7 micrometers) was significantly lower than in the saphenous varicose vein (mean 1609,7 micrometers). The correlational analysis of reflux in SFJ with clinical classification or histopathological finding did not show statistically significant findings. CONCLUSIONS The greater the age, the greater the clinical severity of the patients. The GSV wall is thicker in patients with lower limb varicose veins, but those histopathological changes are not correlated with the disease's clinical severity or reflux in the SFJ on a Doppler ultrasound.


RESUMO OBJETIVO Este estudo tem como objetivo correlacionar os dados demográficos, os diferentes graus clínicos da insuficiência venosa crônica (Ceap), com achados ultrassonográficos de refluxo da junção safenofemoral (JSF) e os achados anatomopatológicos do segmento proximal da veia safena magna (VSM) extraído de pacientes com insuficiência venosa crônica (IVC) primária submetidos à safenectomia magna para correção de varizes dos membros inferiores. MÉTODO Estudo prospectivo de 84 pacientes e 110 membros submetidos à safenectomia magna para o tratamento de varizes de membros inferiores, correlacionando a sua classificação clínica Ceap, presença de refluxo na JSF ao ultrassom Doppler e alterações histopatológicas. Comparamos ainda os achados histopatológicos da VSM proximal retirada dos pacientes com IVC com grupo controle de VSM normal retirada de cadáveres. RESULTADOS Média de idade dos pacientes foi maior nos Ceaps avançados quando comparado Ceap C2 (46,1 anos) com C4 (55,7 anos) e C5-6 (66 anos), e pacientes C3 (50,6 anos) com C5-6. A espessura da parede da VSM normal (média de 839,7 micrômetros) foi significativamente menor do que das VSM varicosas (média de 1.609,7 micrômetros). As análises de correlação da presença do refluxo em JSF com a classificação clínica ou achado histopatológico não demostraram ser estatisticamente significativas. CONCLUSÕES Quanto maior a idade, mais avançada é a classificação clínica da IVC dos pacientes. A espessura da parede da crossa da VSM é maior nos pacientes com IVC e essas alterações não se correlacionam com a classificação clínica da doença ou com a presença de refluxo na JSF ao ultrassom Doppler.

Resumo A maior causa de recidiva das varizes dos membros inferiores é a inadequada dissecção da crossa da safena interna, com a ligadura não rente da junção safenofemoral. O acesso direto ao tecido cicatricial de uma cirurgia prévia deve ser evitado ao máximo pelo elevado risco de sangramento e de lesões linfáticas. O acesso proximal ao tecido cicatricial, abordando inicialmente a veia femoral comum acima da junção safenofemoral, seguindo-a em direção caudal até a crossa da safena, mostrou-se uma técnica eficiente e relativamente simples quando comparada às abordagens medial, lateral e direta.


Abstract The most common cause of recurrence of varicose veins in the lower limbs is inadequate dissection of the internal saphenous arch, with ligature that is not flush to the saphenofemoral junction. Direct access to the scar tissue of previous surgery should be avoided to the maximum because of the high risk of bleeding and of lymphatic injuries. Access proximal to the scar tissue, initially at the common femoral vein, above the saphenofemoral junction, continuing caudally to the saphenous arch, proved to be an effective and relatively simple technique when compared to medial, lateral and direct approaches.

ABSTRACT

OBJECTIVES: This study compared radiofrequency ablation versus conventional surgery in patients who had not undergone previous treatment for bilateral great saphenous vein insufficiency, with each patient serving as his own control. METHOD: This was a randomized controlled trial that included 18 patients and was carried out between November 2013 and May 2015. Each of the lower limbs of each patient was randomly assigned to undergo either radiofrequency ablation or conventional surgery. Clinical features (hyperpigmentation, hematoma, aesthetics, pain, skin burn, nerve injury, and thrombophlebitis) were evaluated at one week, one month, and six months postoperatively. Hemodynamic assessments (presence of resection or occlusion of the great saphenous vein and recurrent reflux in the sapheno-femoral junction and in the great saphenous vein) were performed at one month, six months, and 12 months postoperatively. The independent observer (a physician not involved in the original operation), patient, and duplex ultrasonographer were not made aware of the treatment done in each case. Clinicaltrials.gov: NCT02588911. RESULTS: Among the clinical variables analyzed, only the aesthetic evaluation by the physicians was significant, with radiofrequency ablation being considered better than conventional surgery (average, 0.91 points higher: standard deviation: 0.31; 95% confidence interval: -1.51, -0.30; p=0.003). However, in our study, we observed primary success rates of 80% for radiofrequency ablation and 100% for conventional surgery. CONCLUSIONS: If the physician is not required to inform the patient as to the technique being performed, the patient will not be able to identify the technique based on the signs and symptoms. Our study revealed that both techniques led to high levels of patient satisfaction, but our results favor the choice of conventional surgery over radiofrequency ablation, as patients who underwent conventional surgery had better hemodynamic assessments.

Resumo O tratamento tradicional da insuficiência da veia safena magna (VSM) inclui a ligadura alta na junção safeno-femoral combinada com a fleboextração. No entanto, a morbidade associada à insatisfação do paciente com esse tratamento tem conduzido ao desenvolvimento de técnicas alternativas, e a termoablação com laser endovenoso (EVLT) tornou-se uma alternativa minimamente invasiva à cirurgia. A formação de fístula arteriovenosa (FAV) durante o EVLT é extremamente rara. Neste estudo, relatamos um caso de identificação ecográfica de FAV entre um segmento da veia safena acessória lateral e a artéria femoral superficial. Optou-se inicialmente pela realização de duas tentativas de compressão com transdutor linear, sem sucesso, e alternativamente o procedimento cirúrgico foi realizado sem intercorrência e com resolução da FAV. Esse relato de caso evidencia a importância do seguimento de vigilância ecográfica após o EVLT tanto para o controle da efetividade do método como para o diagnóstico e tratamento precoce de suas complicações.


Abstract Traditional treatment for great saphenous vein (GSV) insufficiency includes high ligation of the saphenofemoral junction and subsequent stripping of the GSV. However, the considerable morbidity and patient dissatisfaction associated with surgical treatment led to development of alternative techniques and intravenous laser treatment (EVLT) has emerged as a minimally invasive alternative to surgery. Formation of arteriovenous fistulas (AVF) during EVLT is extremely rare. In this study we report a case of AVF between a segment of the lateral accessory saphenous vein and the superficial femoral artery that was identified by ultrasound. Initially, two attempts were made at compression with a linear-array transducer without success, then alternatively surgery was performed without complications, leading to resolution of the AVF. This case report highlights the importance of ultrasound follow-up after EVLT, both for monitoring the effectiveness of the method and for diagnosis and early treatment of its complications.

ABSTRACT PURPOSE: To develop an ex vivo model for the analysis of macroscopic, histological and immunohistochemical changes after experimental endovenous laser ablation (EVLA) of the great saphenous vein (GSV). METHODS: We describe a model produced with glass tubes and introducer sheaths to mimic the physiological conditions of EVLA procedures, such as tumescence and blood flow. A pilot study was conducted to evaluate an ex vivo procedure of EVLA of an incompetent GSV segment using a 1470-nm radial fiber diode laser (7 W power) and an automatic pull-back device. The vein segment was analyzed macroscopically and by hematoxylin & eosin staining, elastic fiber histochemistry, Gomori's trichrome staining, and alpha-smooth muscle actin immunohistochemistry. RESULTS: No perforations were observed macroscopically. No muscle cell adhesion was observed in the central part of the ablated vein, showing tissue disruption. There was low labeling for elastic fibers, disruption of muscle fibers, and a reduced expression of the specific marker for this cell type. CONCLUSION: This ex vivo endovenous laser ablation model is a low cost alternative to in vivo experiments, providing standardized experimental conditions.

INTRODUÇÃO: A úlcera venosa assume grande importância na vida dos pacientes, pois a ocorrência de deformidade causada por este tipo de ferida pode gerar consequência advensas as causais inclui distúrbios psicossocial. Avaliar bem-estar subjetivo e depressão em pessoas idosas com úlcera venosa. MÉTODOS: Estudo clínico, primário, descritivo, analítico e multicêntrico. Participaram 55 indivíduos idosos com úlcera venosa, atendidos no Núcleo de Assistência e Ensino em Enfermagem do Hospital das Clínicas Samuel Libânio, no Ambulatório São João da Universidade do Vale do Sapucaí e nas Unidades Básicas de Saúde da cidade de Pouso Alegre. Foram incluídos pacientes com idade acima de 60 anos, ambos os sexos, índice tornozelo/braço entre 0,8 e 1,0. Os dados foram coletados no período compreendido entre dezembro de 2012 a maio de 2013, após aprovação Comitê de Ética em Pesquisa da Faculdade de Ciências da Saúde Dr. José Antônio Garcia Coutinho sob parecer 3.090.46. Os instrumentos utilizados foram: dado sócio demográfico, Escala de Depressão Geriátrica em versão reduzida de Yesavage e Escala de Bem-estar Subjetivo. Para análise estatística foi realizado os testes: Qui-Quadrado, t de Student. RESULTADOS: A Maioria, dos indivíduos, era do sexo feminino, 22(40%) tinham entre 60 e 65 anos, 27(49,10%) entre 66 e 70 anos, tinham de 1 a 2 salários mínimos e viúvo. Foram identificados 23 (41,82%) idosos com úlcera venosa nível da depressão leve ou moderada, e 26(47,28%) depressão severa. Relacionado à Escala Bem-estar Subjetivo a maioria dos participantes do estudo apresentaram alteração nos domínios: 43(78,20%), satisfação com a vida, e 40 (72,70%). CONCLUSÃO: Através deste estudo concluímos que os indivíduos que participaram da pesquisa apresentam depressão entre leve a severa e queda na qualidade de vida. Sentem-se infelizes, e insatisfeitos com a vida.


INTRODUCTION: Venous ulcers play an important role in patients' lives, as the deformities caused by this type of wound can lead to various adverse effects, including psychosocial disorders. OBJECTIVE: To evaluate subjective wellness and depression in elderly patients with venous ulcers. METHODS: This is a primary, descriptive, analytical, and multicenter clinical study involving 55 elderly patients with venous ulcers, undergoing treatment at the Nursing Assistance and Training Center of Clínicas Samuel Libânio Hospital, at the São João Outpatient Clinic of the University of Vale do Sapucaí and at the Primary Healthcare Units of Pouso Alegre city. The study included male and female patients, aged 360 years, and with an ankle-brachial index between 0.8 and 1.0. Data were collected between December 2012 and May 2013, after approval by the research ethics committee of the Faculty of Health Sciences Dr. José Antônio Garcia Coutinho, under recommendation 3.090.46. Sociodemographic data, Yesevage's reduced version of the Geriatric Depression Scale, and the Subjective Wellness Scale were used. Statistical analyses were carried out with the chi-square and Student's t tests. RESULTS: Most of the study participants were female; 22 (40%) of the patients were aged 60-65 years and 27 (49.10%) were aged 66-70 years. Their income ranged between 1 and 2 minimum wages, and most of them are widowers. Twenty-three (41.82%) elderly patients with venous ulcers had mild to moderate depression levels, and 26 (47.28%) showed severe depression. In the Subjective Wellness Scale, most study participants revealed changes in items 43 (satisfaction with life; 78.20%) and 40 (72.70%). CONCLUSION: This study revealed that study participants had mild to severe depression and decreased quality of life.

PURPOSE: To assess venous hemodynamics and quality of life at lest five years after varicose vein stripping. METHODS: We conducted a prospective study with 39 patients (63 limbs) with primary lower limbs varicose veins. Preoperatively, all patients were subjected to clinical evaluation, duplex ultrassound, air plethysmography (APG), and CIVIQ questionnaire of quality of life. By APG, venous filling index (VFI), ejection fraction (EF), and residual volume fraction (RVF) were determined. CIVIQ addresses four domains in course of 20 questions, including states of physical, social, and psychological well-being, and pain level. Varicose veins were treated by standard venous stripping. At least five years after surgery (77.9 ± 10.9 months), patients were reassessed and had clinical examination, duplex ultrasound, APG, and CIVIQ repeated. No late follow-up data was available for 24 patients. RESULTS: Preoperative and late postoperative VFI levels were similar, whereas EF (P=0.05) and RVF (P=0.01), as hemodynamic variables, significantly improved following surgery. In the late postoperative period, overall CIVIQ scores were significantly lower (P=0.005), as were scores in all four domains: pain (P=0.001), physical (P=0.007), social (P=0.008), and psychological (P= 0.05). CONCLUSION: In a small prospective cohort, improvements in venous hemodynamics and in quality of life of patients submitted to standard varicose veins stripping were maintained five years after the procedure.

ABSTRACT

A síndrome de congestão pélvica é definida por critérios anatômicos, hemodinâmicos e clínicos, decorrentes da hipertensão e estase venocapilar no território pélvico, podendo ser estendido aos membros inferiores. A presença de varizes dos membros inferiores pode estar ou não relacionada ao refluxo do sistema safeno e, nos casos de ausência do refluxo safeno, o refluxo pélvico apresenta-se como importante fator etiológico responsável pelo aparecimento e recidiva da doença venosa nos membros inferiores. Os autores relatam uma pequena série de três casos de pacientes jovens portadoras de síndrome de congestão pélvica associada à varizes dos membros inferiores tratadas por embolização pélvica e tratamento cirúrgico das varizes de membros inferiores, no mesmo tempo cirúrgico, com bons resultados iniciais e ausência de complicações embólicas.


The pelvic congestion syndrome is defined by anatomical criteria, hemodynamic and clinical result of hypertension and venocapillary stasis in the pelvic area and can be extended to the lower limbs. The presence of varicose veins of the lower limbs may or may not correlate reflux of the saphenous system and, in cases of absence of saphenous reflux, the reflux pelvic presents itself as an important etiological factor responsible for recurrence of venous disease in lower limbs. The authors report a small series of three cases of young patients suffering from pelvic congestion syndrome associated with varicose veins of the lower limbs treated by pelvic embolization and surgical treatment of varicose veins of the lower limbs during the surgical procedure with good initial results and no embolic complications.

CONTEXTO: Apesar de reconhecer que o número apresentado pelo mutirão de cirurgias eletivas não refletisse inteiramente a realidade, foi significativa a implementação financeira do Ministério da Saúde do Brasil, bem como o aumento de cirurgias de varizes, após criação do Fundo de Ações Estratégias e Compensação (FAEC). OBJETIVO: Avaliar os resultados da aplicação do investimento financeiro do Ministério da Saúde no mutirão de cirurgias de varizes. MÉTODOS:Foi realizado um estudo transversal de natureza retrospectiva, utilizando informações do banco de dados do Departamento de Informática do Sistema Único de Saúde. Foram incluídas na pesquisa todas autorizações de internação hospitalar das cirurgias de varizes de membros inferiores pagas pelo Ministério da Saúde do Brasil, no período de janeiro de 1998 a dezembro de 2004. RESULTADOS: Em 1998, antes da implementação do mutirão de cirurgias eletivas foram realizadas 23.531 cirurgias de varizes e investidos R$ 5.819.033,27. Após a criação do FAEC, foram realizadas 457.026 cirurgias de varizes de membros inferiores, no período de 1999 a 2004, e foram investidos R$ 187.760.196,81 com média de R$ 31.293.336,13 por ano. CONCLUSÃO: O investimento feito pelo Ministério da Saúde durante o programa de mutirões de cirurgias eletivas pelo FAEC, proporcionou aumento significativo do número de cirurgias de varizes em todo Brasil.


BACKGROUND: In spite of the fact that the number of surgeries presented by the elective surgery task force did not entirely reflect the reality, it is possible to declare that the financial implementation of the Brazilian Ministry of Health has been significative, as well as the increasing number of varicose vein surgeries, especially after the creation of the Strategic Actions and Compensation Fund (FAEC). OBJECTIVE: To evaluate the application of financial investment in the Ministry of Health campaign for varicose vein surgery. METHODS: A transversal study of retrospective nature has been conducted, using information available at the Data Processing Department of the National Healthcare System database (DATASUS). All the authorization of hospital internment from the inferior member varicose vein surgeries, financed by the Brazilian Ministry of Health from January 1998 to December 2004, have been included in the research. RESULTS: In 1998, before the implementation of the elective surgery task force, 23,531 varicose vein surgeries have been performed, and R$ 5,819,033.27 invested. After the creation of the FAEC, 457,026 inferior member varicose vein surgeries have been performed from 1999 to 2004, and R$ 187,760,196.81 were invested with an average of R$ 31,293,336.13 per year. CONCLUSION: Thus, it is possible to conclude that the bigger investment from the Brazilian Ministry of Health, represented by the implementation of the elective surgery task forces by the FAEC, has been responsible for increasing the number of varicose vein surgeries all around Brazil.

ABSTRACT

Objetivo: comunicar nuestra experiencia en eldesarrollo de la flebectomía venosa.Materiales y métodos: entre Marzo de 2007 y Abrilde 2009 en el Servicio de Flebología y Linfología delHospital Nacional de Clínicas de la ciudad de Córdoba,se realizó un estudio con el objetivo de describir la técni-ca quirúrgica de la mini-flebectomía venosa, se evalua-ron ade m ás los resu ltad os, las com p lica cion espostoperatorias y la tasa de recidivas. Resultados: Latasa de recidivas fue de 0% para el sector venoso inter-venido. Con respecto a las complicaciones encontra-mos un 1,68% de pigmentación en el sitio de la incisión,hematomas en un 2,52%, parestesias en un 3,36% delos casos, infección local en 0,84% de los pacientes,induración subcutánea en 6,72% de los casos, depre-sión sobre la cicatriz en 0,84% y no se presentaron ca-sos de cicatriz dolorosa. Conclusiones: Nos encontra-mos ante la presencia de un método extensamente pro-bado en su eficacia, seguridad, inocuidad, con una ele-vada tasa de satisfacción para los pacientes, que permi-te una rápida reinserción laboral, con una tasa despre-ciable de complicaciones.

With regard to complications we found a 1.68% of pigmentation at the site of the incision,bruising or 2.52%, 3.36% paresthesias cases, local infection in 0.84% of patients, subcutaneous induration6.72% of cases, depression of the scar in 0.84% andthere were no cases of painful scar. Conclusions: We arein the presence of an extensively tested methodeffectiveness, safety, safety, with a high rate of satisfactionfor patients, allowing a quick return to work, with anegligible rate of complications.
